To all Members in the Voter's List
Please bring the following for proper identification and verification
- For Homeowners:
- Valid government-issued identification card with picture and signature
- Copy of Marriage Contract/License or Title of the property (if property is in the name of the spouse)
- For Corporate members:
- Photocopy of Registration with SEC, DTI, HLURB, etc
- Original copy of notarized Board Resolution or Secretary's Certificate authorizing the Authorized Representtive to vote.
- For Authorized Representatives:
- Valid government-issued identification card with picture and signature
- Photocopy of Member's/Property owner's valid government-issued identification card with picture and signature of
- Original copy of notarized Special Power of Attorney authorizing holder to vote
- For Proxies:
- Valid government-issued identification card with picture and signature
- Photocopy of Member's/Property owner's valid government-issued identification card with picture and signature of
- Proxy Form with original signature of Member/property owner.
